COMMUNITY GUIDELINES
GENERAL LIABILITY WAIVER FORM NOTES:
Failure of the athlete(s) to fill out our General Liability Waiver Form, due to non-compliance, amnesia, and/or negligence will result in the player:
1) Being requested to FILL OUT WAIVER FORM.
2) If the athlete(s) continues to play and/or becomes injured while playing, the athlete accepts FULL RESPONSIBILITY for medical coverage and/or litigation of that athlete.
3) Absolutely NO REFUNDS.
By reserving ATLAS Hoops or participating in open runs, basketball training, performance training, basketball camps, and/or physical therapist, the participant(s) or reservee, or persons involved with the reservee’s party, hereby acknowledge and understand the inherent risks associated with participating in basketball activities at ATLAS Hoops. By entering and using the facilities, I voluntarily assume all risks, hazards, and dangers associated with such activities. I release ATLAS Hoops, its owners, employees, and affiliates from any and all liabilities, claims, demands, actions, or causes of action whatsoever arising out of any damages, injuries, or losses sustained by me while on the premises. I certify that I am in good health and have no physical conditions that would prevent my safe participation. I further agree to abide by all rules and regulations of ATLAS Hoops during my visit.

Equipment Damage:
Equipment damage is subject to the FULL market value price of the item.
Wall Damage:
< 1 ft x 1 ft in size = $100
> 1 ft x 1 ft in size = $200
TEMPERATURE:
SUMMER:

As the Las Vegas temperatures increase, the facility is warm (varies between 78 – 88° F). We are cooled, via EVAPORATIVE COOLERS, which cool the facility 15-25° cooler than it is outside (ie. Outside Temperature: 95°; Court Temperature: 75°). There is a water station located in the lobby and water and Gatorade are available for purchase; however, it is STRONGLY advised to stay hydrated before, during, and after your basketball session. By signing up, you understand these terms.
FALL/WINTER:

As the Las Vegas temperatures cool off, the facility becomes cooler (varies between 65-75° F). We provide heating stations (radiant heaters) in several spots within the gym. In addition, the air becomes increasingly dry and with the bouncing of leather and/or rubber basketballs results in an increased amount of static shock within the gym as you touch metal objects. Unfortunately, there is nothing we can do about the air dryness and we advice that you use towel when touching, opening, and/or closing metal objects.
RESERVATION EXTENSION:
Any time extension(s) to your reservation time must be communicated >24 hours before reservation for no charge to occur.
Requests for EXTENSION OF YOUR RESERVATION TIME, DURING OR AT THE END OF YOUR RESERVATION, will result in an $85 per hour FULL-COURT RESERVATION SURCHARGE.
TRAINING/PRACTICE:
Basketball Trainers:
It is recommended that you obtain your own basketball insurance. By completing single payment for half or full court (or monthly retainer), you accept FULL, COMPLETE responsibility of your player(s) if a) they are injured under your supervision and b) even if you, the trainer, has no basketball insurance coverage.
Middle School/High School/College/Prep/AAU/Club Teams:
It is recommended that you obtain your own basketball insurance for player and team coverage. By completing single payment for half or full court (or monthly retainer), you accept FULL, COMPLETE responsibility of your player(s) if a) they are injured under your supervision and b) even if you, the team, has no basketball insurance coverage.
CANCELLATION:
Reservation(s) may be canceled at the Owner’s Discretion. If owner CANCELS at their discretion, FULL REFUND will be PROVIDED.

PROMOTIONAL, PHOTO SHOOTS, BRANDING PURPOSES:
For branding, promotional, and photo shoot purposes, full privacy and use of the entire facility (except gym office) is $1000/hour. For booking for purposes, as stated above, please provide media deck, party members, and how ATLAS will be represented in the media to atlashoops702@gmail.com.
OFF-BUSINESS RESERVATIONS:
Reservation outside ATLAS business hours will incur an additional Surcharge Fee of $15/hour (Full Court: $85/hour and Half Court: $55/hour).
GYM USAGE:
Gym usage is only for the party that reserves the space. For “profit” or “reselling” gym time at a higher price, is STRICTLY PROHIBITED and will result in immediate CANCELATION and NO REFUND for your reservation.
